How many residents live at Evergreen Haven?
We care for no more than six residents at a time. This small setting allows us to keep a one-to-three caregiver-to-resident ratio and give each resident truly personal attention.

We create a customized care plan for every resident. Our team supports:

  • Dementia and memory care
  • Diabetes management, including insulin administration
  • Parkinson’s and other chronic conditions
  • Incontinence support
  • Medication administration and coordination with doctors and pharmacies
  • End-of-life / hospice care

If your loved one has unique medical needs, we coordinate closely with physicians, home-health agencies and therapy providers.

Yes. Every resident has a private bedroom, some with their own bathroom, and may personalize it with furniture, photos, or keepsakes.

Rooms are staged with furniture that can be kept or removed based on preferences, and several rooms include full private bathrooms.

Our rates are all-inclusive and cover:

  • 24/7 caregiver support
  • Home-cooked meals and snacks
  • Housekeeping and laundry
  • Medication management
  • All scheduled activities and life-enrichment programs
  • Basic utilities and room maintenance

There are no hidden “tiered add-ons,” and we do not raise rates mid-year. We are always happy to walk through pricing in detail so there are no surprises.

We accept:

  • Private Pay
  • 3 Year Medicaid conversion for residents who plan to spend down over time

We are happy to walk families through the process.

There are no formal visiting hours. Families and close friends are welcome anytime. We just ask that late evening visits or visits during scheduled medical care be coordinated with our staff so we can make sure your time together is comfortable.

Absolutely. We encourage families to participate in gardening days, baking afternoons or simply to share a meal. Staying connected to loved ones is an important part of life at Evergreen Haven.

You can call or text us directly—there is no front desk or phone tree. We also provide regular updates and photos and will contact you immediately with any changes in health or care plans.

  2. Assessment – we meet your loved one and review medical history and care needs.
  3. Care plan & room selection – we finalize the plan and confirm the room.
  4. Move in – we help with orientation and settling in.
Personal clothing, toiletries and any special items that make the room feel like home—a favorite chair, wall art or keepsakes. We provide furniture if needed, but personal touches are welcome.

We have a 24/7 on-site caregiver team, an NFPA 13D fire-sprinkler system, secure door alarms, and detailed emergency plans. In a medical emergency we contact family immediately and coordinate with local hospitals and physicians.

Yes. We work closely with local hospice agencies and provide full support so residents can remain comfortable and families can focus on time together.
Meals are home-cooked and tailored to each resident’s needs. We accommodate diabetic, renal and low-sodium diets and are happy to discuss any specific dietary requests.

For the health and comfort of all residents, we do not allow live-in personal pets, but therapy animals and visiting pets are welcome during family visits.
